At Evergreen we believe every person deserves a better healthcare experience.
We partner with nephrologists to provide comprehensive care that goes beyond kidney disease and addresses the patient as a whole including the emotional, spiritual, mental, and social aspects of their lives.
We believe that by focusing on our patients' complete care needs through a value-based care model, we can change the trajectory of healthcare.
“Over 37M Americans have kidney disease, and it is the ninth leading cause of death in the United States. This is unacceptable and why we created Evergreen.”
We serve people. People who love to cook Sunday night dinner with their family or order takeout from their favorite local restaurant. People who challenge their grandchildren to a game of cards or challenge their brain with the latest crossword puzzle.
We serve people who love to go for walks outside and those who would rather stay inside with their cat. People who love to have their hands in the dirt of their vegetable garden or fall asleep holding the hand of someone they love. We serve people who love to tinker under the hood of their favorite car and those who would rather sit on the front porch and watch the cars go by.
These people have Chronic Kidney Disease – but that is not what makes them who they are.
We are dedicated to caring for people on a more personal level, helping them feel whole again, in the face of chronic illness. Ultimately, we can change the trajectory of healthcare – one person at a time.
Our Interdisciplinary Care Team (IDT) makes this come alive.
We hire several clinical roles, click here to see the care team you’d work alongside.
Behind the scenes, our corporate staff of accounting experts, actuaries, analysts, central operations roles, and more enables our field team to do their best work.
Though not every role is live in all markets, this visual shows you our team approach to whole-person care, and the wrap around services our IDT provides.

We value continuous learning.
We recently introduced our Culture Code. The goals we set out to achieve may seem lofty, and that’s on purpose. We cannot change the trajectory of healthcare without aiming high.
Kidney disease disproportionately impacts black and brown communities, immigrant communities, and low-income communities – many of whom overlap. Therefore driving population health with a specific focus on improving healthcare equity is paramount to our success.
We believe advocating for diversity, equity, inclusion, and belonging — just as building culture — is everyone’s responsibility. Making healthcare more human means recognizing everyone for what makes them uniquely them.
