We are putting nephrologists in the driver’s seat, where they belong.

We help nephrologists focus on the right patients at the right time across the full spectrum of care. Through strategic partnerships, we empower nephrologists with financial backing to implement best-in-class interdisciplinary clinical resources, analytical insight and tools, and services to patients.

When nephrologists are empowered, they can begin to transform kidney care and work with their patients earlier to identify alternative modalities, slow disease progression, improve clinical outcomes… and ultimately improve the quality of their lives.

We are committed to providing high quality kidney care and improving the experience for both providers and their patients.

Our technology-enabled partnership addresses barriers to care transformation...

Disparate EHRs for CKD and ESRD patients
Lack of information regarding transplant status and process
Difficulty in accurately documenting patients’ CKD and ESRD status in real-time
Difficulty in predicting and tracking which patients will develop ESRD
Insufficient resources and tools to target and de-risk readmission

A Mission with Meaning

Evergreen’s founding team is passionate about empowering nephrologists to create an improved experience for people living with chronic kidney disease. Our team not only have years of kidney care experience, but we have also been touched personally by kidney disease.

Evergreen Nephrology’s Co-Founder and Executive Chairman Adam Boehler drove efforts at the Centers for Medicare & Medicaid Services (CMS) Innovation Center (CMMI) and the department of Health and Human Services (HHS) to introduce the kidney care models (KCC) that empower nephrologists as they care for people living with CKD and ESRD. In addition to transforming dialysis delivery, the HHS team set new standards for organ procurement to increase transplantation. Adam and fellow Co-Founder Abe Sutton were awarded the 2019 President’s Medal from the American Society of Nephrology.
